Primary Care – Geriatric Physician (PACE Program)

 

PACE Program (Program of All-inclusive Care for Elderly) is currently seeking a Physician/Geriatrician.  This is one of 134 PACE programs in the nation, is an innovative, comprehensive care management model serving frail elders aged 55 and above. Program is seeking a Physician to join our current staff of four geriatricians, 4 geriatric nurse practitioners, and a psychiatric nurse practitioner.

 

The elements of PACE model of care include an 11 member Interdisciplinary Team which manages the care of frail elders and supports each other and the participant in achieving care goals. Participants attend the center for Adult Day Health activities as well as receive on-site clinical care including primary care, behavioral health, and rehabilitation.  Program  physicians follow the elders throughout all locations of care including acute, sub-acute, long-term, and home care settings. Small Patient panel (approximately 150 patients per Clinical Team). Regular home visits to our participants are encouraged and time is allotted for this effort. 

 

PACE is a completely capitated program which allows us the flexibility and creativity to provide the best, individualized care for patients. Quality Improvement efforts are a key element of our care model.  We are seeking a Physician with a completed accredited residency in Geriatric Fellowship training, although we would consider a Physician board certified in Internal Medicine or Family Practice with extensive experience working with the Geriatric population.
