 FNP Opportunity at Outpatient Advanced Primary Care Clinic 

We seek a skilled and compassionate Family Nurse Practitioner (FNP) to join a new advanced primary care health center in Liverpool, NY. This is a solo provider site, supported by one Medical Assistant (MA), and will serve a patient population of 300-500 individuals. The role involves managing a range of health conditions, with a focus on chronic disease management and some occupational medicine (including DOT physicals). 

We are looking for someone who is comfortable working independently and providing care for patients ages 3 and up. The role offers flexibility in scheduling, and we are committed to accommodating provider preferences where possible. 

Key Responsibilities: 

·         Provide primary care and chronic disease management 

·         Conduct DOT physicals and other occupational health services 

·         Work collaboratively with the Medical Assistant to ensure high-quality patient care 

Key Highlights: 

·         Monday - Friday (flexible, typically 3 regular days 8:00 AM - 4:00 PM, 1 earlier start at 7:00 AM, and 1 later shift until 6:00 - 7:00 PM) 

·         Smaller patient panel (less than half the traditional PCP panel size) 

·         Focus on outcomes, NOT production 

·         Competitive salary range of $130,000 - $140,000

Qualifications: 

·         Licensed Nurse Practitioner (NP) 

·         Experience in chronic disease management and occupational health preferred
