Nurse Practitioner for Care Coordination Center

Nurse Practitioner (NP) needed to join comprehensive care team. NP will be mainly responsible for diagnosing and treating patients independently and in collaboration with physician(s) and APP colleagues.The NP will facilitate the continuity of patient care by facilitating and coordinating communication between the health care team, patient and patients’ family.The goal is to serve as a link to integrating relevant services both internal and community based consistent with the Institute for Healthcare Improvement’s triple aim of better health, better care, and lower cost. 

Successful candidate will be a graduate of a Master’s degree program with concentrated area of study in acute, or adult medicine. Current licensure in Maryland and ACLS and BCLS certification required.Position will require three-years of previously worked acute care NP experience or experience with chronic illness and or other accepted related experience as appropriate. 

The position is day shift Monday – Friday 8am - 4:30 pm. Competitive salary and full benefits package included.
