THIS POSITION IS A STATE TRAVELER POSITION-Based on the business needs, this position would require the ability to travel as needed for extended periods of time.

MUST HAVE A MINIMUM OF 1 YEAR NP EXPERIENCE.

M-F, No Call, and WORK/LIFE/BALANCE!
Perform Health Risk Assessments on geriatric patients in their homes, local nursing home, and skilled nursing facilities
See approximately 6 patients per day autonomously
Join a team of Nurse Practitioners whose collaborative care goal is to improve the healthcare of geriatrics, and reduce inappropriate hospitalizations

To join, you will need:

1 years Nurse Practitioner experience required 
Current NP licensure in state of residence.
Board Certified Nurse Practitioner with specialty in Adult, Family or Geriatric Care
